A word of advice..Don't give up..I was persistent.. I even had to show the judge I was in a body cast after a fusion back surgery, I believe it was searching for back pain relief surgery number 4...the moral of the story is I SUCCEEDED I am not sure how long it took

 It has all kinds of different information on Social Security Disability Benefits, including the evaluation form.

I received retroactive payments for social security disability, dated  5/5/ 1985,  the date my doctor sent in my papers to work about being disabled due to chronic back pain. I also received..back payments and benefits for my 2 sons at the time John was 10 and David was only 3 years old. I received payments for both boys until they graduated high school.

No matter what you do don't give up, I know it is a very long, seemingly unreachable goal to reach,
